My Scorecard
The page where an employee fills in their own performance assessment. Available once the scorecard reaches the Active stage.

The indicator list can be shown as cards (the default on this page — one card per indicator) or as a table (ruled columns, for scanning every number at once). The toggle sits at the top right of the list; both open the same window for entering a result or reviewing evidence.
An indicator labelled Complement sits directly beneath the one it completes — the two are a single measure split in two, sharing one weight between them. It is still a full indicator as far as you are concerned: it takes its own result and its own evidence, like any other.
Step by step
Choose the current quarter.
For every indicator, enter the result — the figure genuinely achieved, in that indicator's unit. All of them, not some: an indicator left blank counts as zero, not as skipped.
Click Evidence on that indicator and attach a file. PDF, JPG, or PNG, maximum 10 MB. At least one piece of evidence per indicator filled in.
Complete the self-assessment section.
Click Submit.
Once submitted, you cannot change it
It moves to Manager Review. The only way back is for your manager to return it — and in that case you will receive their reason.
Check it over before pressing submit.
You do not fill in Form 2
The behaviour section is assessed by your manager, and appears as a locked row on your screen.
What commonly blocks submission

Collect evidence through the quarter, not at the end
Evidence hunted down on the last day is usually whatever came to hand. Attach it as soon as the figure exists — this page can be filled in gradually, not in one sitting.
Also available on the phone
The Kreatime app offers the same screen: enter results, attach evidence straight from the camera or gallery, read the manager's behaviour assessment, follow the approval history, and submit for review.
There is also an Annual Result screen: the year's score, the original grade, the calibrated grade (once results are locked), and the per-quarter breakdown.
The rules are identical to the web version — the system enforces them, not the app, so neither side offers a shortcut.
